A Time of Transition DVD (ALF, SNF)
A Time of Transition has been designed to help facilities better communicate the realities of nursing home life. It is intended to be shown to a resident's family members or other responsible parties upon admission.
The two main goals of the DVD are to introduce families to the realities of nursing home life—what new residents and families can expect as their loved one transitions to their new life—and to reassure families that they made the right decision in admitting their loved one to a nursing home.
This DVD is intended to be the first step in an on-going process of communication with family members and responsible parties. If used correctly, this DVD will help a facility portray a realistic and positive portrait of life in a nursing home.
This DVD is produced by the Ohio Healthcare Association.
